Children entering Kindergarten, Third grade, Seventh grade, Ninth grade, Twelfth grade, or who are new in the state of Iowa are required to submit the forms below. Please look at your child's grade level to determine what is needed. Print out the needed forms and take them with you to your child's appointments.

Kindergarten Requirements
Students entering Kindergarten are required to have the following:
Physical- completed within 1 calendar year of the first day of school. Many doctor's offices have their own form or will print out a summary of their well child visit- both are acceptable.
Health History Form- is not a state requirement but provides the nurse with a background of your student's health. It is attached below.
Lead Test- a Blood Lead Level test is required by the state of Iowa. This test may have been completed at an earlier age.
Immunizations- by state law a copy of your child's immunizations are required by the first day of school or your child will be unable to attend. Children need the following immunizations: 5 doses of DTap, 4 doses of Polio/IPV, 3 doses of Hepatitis B, 2 doses of MMR, and 2 doses of Varicella/ Chickenpox. These doses need to be spaced appropriately. Please see the attached Immunization Requirements file for more information and the Certificate of Immunizations form to see the document that is returned to the school by the first day of the school year. If your child is behind on their immunizations a Provisional Certificate will be completed and they will be considered to be on a schedule for immunizations. Documents for parents wishing to exempt out of immunizations due to a religious or medical reason are also below and need to be turned in by the student's first day of school.
Vision Screening - a vision screening or exam should be performed no earlier than 1 year prior to the start of Kindergartner and no later than 6 months after the start of the school year. A green vision exam card will be provided in the Kindergarten Round-Up health packet and the vision screening form is attached below.
Dental Screening- all children newly enrolling in an Iowa elementary school are required by law to have a dental screening. The screening may be performed by a licensed dentist, dental hygienist, physician, nurse, advanced registered nurse practitioner, or physician assistant. The screening must have been performed no earlier than 1 year prior to the start of the school year and no later than 4 months after the first day of school. The dental screening form is attached below.

Third Grade Requirement
Each student in 3rd grade is required to submit a valid vision screening performed no earlier than 1 year prior to enrollment and no more than 6 months after the date of the child’s enrollment. Vision screening form is attached below.

Seventh Grade Requirements
Every student entering 7th grade in Iowa is required to have 1 dose of tetanus, diphtheria, acellular pertussis- Tdap and 1 dose of meningococcal conjugate vaccine (A,C,W,Y)- MCV prior to the start of the school year. Students will not be allowed to attend school until this requirement is met.

Ninth Grade Requirement
Students enrolled in ninth grade are required to submit proof of a dental screening that took place no earlier than 1 year prior to the start of the school year and no later than 4 months after the first day of school. The screening must only be completed by a dentist or dental hygienist.

New Student Health Requirements
Students who are new to the state of Iowa should submit a copy of their most recent physical and immunizations prior to their first day of attendance. Please fill out the health history form which helps the nurse to have a more complete picture of your student's health history.
